Coach Evaluations

Coach evaluations are an important part of our player and coach development. We encourage Association Parents to complete the Coach Evaluation form. This year those that complete the evaluations (with their name) and send them into the SPMHA office will be entered into a draw to receive a 50% discount on next year’s registration (for one player).

We have added a line to put your name on the form (optional). The evaluations remain in confidence and evaluator names are never shared with coaches. However, if you prefer to stay anonymous to the review committee and still have the opportunity to enter the 50% off registration draw, leave your name off the form and do one of the following:

Email response - we will enter the name from the email, but not attach it to the evaluation.

Faxed evaluation - include a cover sheet with your name and contact info and we will enter your name in the draw without attaching it to the evaluation.

In Person – include a cover sheet with your name and contact info and we will enter your name in the draw without attaching it to the evaluation.

We will be making the draw on May 10, 2010.
