2010-2011 Registration
Sherwood Park Minor Hockey Association Registration Details for 2010 – 2011 Season
Any registrations received after July 1, 2010 are considered Late Registrations and are subject to a $100 late fee.
All registrations after July 1st are subject to a waiting list.
***Please read these instructions carefully as the process for registration may have changed for your player for the upcoming season.***
Your registration will fall into one of the following categories:
1. New Registration (new to Sherwood Park)
2. Female players that reside outside the boundaries of Sherwood Park
3. Returning player (Male & Female who played in SPMHA last year and live within our Boundaries)
1. New registration (new to Sherwood Park)
If you are a new player registering for hockey for the first time or you are new to Sherwood Park and registering in SPMHA for the first time, you are required to manually complete the form <registration form> and late registrations must fill out the registration forms 2 copies and submit to the office with the non refundable $100 late fee.
. In addition, you are required to provide your Alberta Health Care #, Birth Certificate and proof of residency along with your 2 copies of your registration. If you have played with a different Association before but are new to SPMHA please complete and bring the Parent Declaration form <parent declaration>.
2. Female players that reside outside the boundaries of Sherwood Park
If you do not reside in Sherwood Park, and wish to register in the female program, you are required to manually complete the form <registration form> late registrations must fill out the registration forms 2 copies and submit to the office with the non refundable $100 late fee.
. In addition, you are required to provide your Alberta Health Care # along with your 2 copies of your registration.
*Important* You must provide, at registration, approval from the Association that is tied to your residence to try-out in Sherwood Park. This Permission to Try-Out form “approval” must be presented at registration and signed by your President <tryout approval>
3. Returning Player (Male and Female who played in SPMHA last year and live within our Boundaries)
If you are a returning player (Male or Female) residing in Sherwood Park you are required to manually complete the form <registration form> late registrations must fill out the registration forms 2 copies and submit to the office with the non refundable $100 late fee.

Fundraising
As with last year, you will be asked to participate in the main fundraising activity of the Association. This is a general “raffle” that sees 70% of the proceeds going to the Association to support the program and 30% of the proceeds going to cash prizes for the winning ticket holders. More details will be available at registration.
Payment Options
For a listing of Registration Fees see the following link <registration form>. Payment for the 2010 – 2011 season is either cash or cheque. SPMHA provides the option of installments for registration fees (details available at registration).
